DACC has launched a Home Visitor Scheme. This reaches out into the community and offers a home visit to anyone aged 60 and over who is housebound, lonely and isolated.

 Home Visitor Scheme Pilot 2023

Our findings show:

    New title
  • In the first 12 months, 46 people were referred by GP Social Prescribing teams as lonely and needing support .
  • 547 home visits were made in the first 12 months.
  • 210 calls made.
  • Average age is 83 year old.


Key Themes included;

  • Loneliness
  • Poor Mental health, suicidal
  • Low Mood,
  • Communication issues
  • Bereavement/Grief
  • Struggling with practical tasks such as telephone and making appointments.
  • Food shopping
