DACC believes in supporting the regeneration of our villages across County Durham, that is why we deliver our day care using local community venues and employ local people wherever possible!

Home Visitor Scheme Pilot 2023

Our findings show:

New title

In the first 12 months, 46 people were referred by GP Social Prescribing teams as lonely and needing support .
547 home visits were made in the first 12 months.
210 calls made.
Average age is 83 year old.

Key Themes included;

Loneliness
Poor Mental health, suicidal
Low Mood,
Communication issues
Bereavement/Grief
Struggling with practical tasks such as telephone and making appointments.
Food shopping
